Dosing and Titration

Doses range from 1.5mg to 25mg, with patients stepping up gradually:

  • Start at 1.5mg, remain for at least one month before increasing
  • Titrate upward through 4mg, 9mg, and eventually 25mg
  • Patients already on 2.4mg semaglutide injections can switch directly to the 25mg tablet

Slow titration reduces the risk of side effects-particularly nausea-which are most common in the first few weeks as your body adjusts.

Side Effects to Know About

Oral semaglutide shares a similar side-effect profile with the injectable form. Most are gastrointestinal and tend to ease as the body adapts:

Common (usually temporary):

  • Nausea and vomiting
  • Diarrhoea or constipation
  • Stomach discomfort and bloating
  • Heartburn or reflux
  • Fatigue or dizziness

Serious (seek medical attention):

  • Severe abdominal pain (possible pancreatitis)

  • Signs of gallbladder issues (upper stomach pain, fever, jaundice)
  • Mood changes or depression

As with all prescription medications, the Wegovy pill is a prescription-only medicine and should only be started under clinical supervision.

Is the Wegovy Pill Available on the NHS?

What's Coming Next: Orforglipron (Foundayo)

The Wegovy pill won't be alone for long. Eli Lilly-the manufacturer of Mounjaro-has developed a rival oral GLP-1 called Orforglipron, marketed as Foundayo.

Professor Naveed Sattar, University of Glasgow: "The approval of the once-daily oral form of Wegovy is welcome news for people living with obesity, particularly those who would prefer not to use injections."

Dr Simon Cork, Anglia Ruskin University, noted that oral formulations are likely to be more attractive but reminded patients that clinical trials show "slightly inferior levels of weight loss compared to injectable forms, particularly Mounjaro."
